Prevocational Progress Review Forms

Each network and hospital will have its own specific requirements for handling these forms, but in general, IMET recommends the following:

Handing out forms:

Mid Term appraisal forms should be given to trainees at the beginning of each term.
The End of Term assessment form should be given to trainees in the middle of term when they hand in their Mid Term forms.

Completing forms:

Term Supervisors should discuss the Progress Review forms with trainees as part of the term orientation.

Trainees should complete their part of the Mid Term form before the mid term meeting with the Term Supervisor.

At the mid term meeting, the Supervisor will discuss the trainee’s progress so far and complete the Supervisor’s part of the Mid Term form.

Trainees should complete their part of the End of Term form before the end of term meeting with the Term Supervisor.

At the end of term meeting, the Supervisor will review the trainee’s achievement in the term and complete the Supervisor’s part of the End of Term form.

Returning forms:

Copies of completed forms should be kept by the trainee and the JMO Management Unit.

Originals collected by the JMO Management Unit must be sent to IMET.

After processing, IMET will return forms and electronic copies to the JMO Management Unit.
